Coordinating Paediatric and Orthodontic Care in Early First Molar Loss, Ectopic Eruption and Impacted Teeth 

Abstract: 
Early disruption of the first permanent molar, ectopic eruption and impacted teeth can profoundly influence occlusal development, craniofacial growth and long‑term oral health. This lecture synthesizes contemporary evidence to support coordinated paediatric–orthodontic decision-making grounded in three-dimensional skeletal, dentoalveolar and facial assessment. 

For early first permanent molar loss, we will review timing considerations, a structured decision-making algorithm, biomechanics of molar protraction, and implications for three-dimensional anchorage control and arch form.

Management of ectopic eruption will encompass diagnostic criteria and risk stratification, interceptive disimpaction techniques, guided eruption protocols, and evidence-based indications for strategic extraction within differing skeletal and vertical patterns.

Approaches to impacted incisors and canines will emphasize three-dimensional diagnosis (CBCT-based trajectory and root risk analysis), surgical–orthodontic exposure protocols, and a critical appraisal of the risk–benefit profile of active alignment versus elective non-exposure, autotransplantation, or acceptance of impaction. 

Throughout, the lecture will integrate longitudinal outcome data and growth prediction to evaluate effects on occlusal stability, periodontal health, and facial esthetics, translating these insights into practical chairside algorithms and “how-to” / “when-not-to” clinical pathways. 

Ultimately, this session will challenge clinicians to calibrate intervention with restraint: discerning when to actively modify the developing occlusion, when to do less to avoid iatrogenic harm, and when to deliberately “let growth do the work,” achieving a scientifically grounded balance between necessary interception and respectful non-intervention in the growing child!
